Case managers, in consultation with consumers, tailor individual direct support and access to quality services for the aged, younger people with disabilities and their carers from all cultural backgrounds who are living at home and whose cultural/language barriers present significant difficulty in accessing mainstream services. Assistance to recipients with home duties, home maintenance, personal care, transport, visits to hospital, doctors, dentists etc. Access to specialist services (physiotherapy, podiatry, hydrotherapy etc) and social support to maintain/improve the person's lifestyle.
