About Extended Aged Care At Home Program (EACH) - Inner West

The EACH Program is designed to enable the older person with high care needs living in the community, to be cared for in their own home by providing a tailor made package to meet their needs. The EACH Package is for people who live alone or with a carer and who need assistance with a variety of daily living skills to enable them to remain at home. The Program can be seen as a bridge to high level care facilities. The client will be cared by care workers, enrolled nurses and registered nurses. Services include, Personal Care, Continence Management, Wound Management, Health and Medication Monitoring, Pain Management, Meal Preparation, Equipment Hire, Domestic Assistance, Respite Care, Pastoral Care, Linen Service and Advocacy. Uniting Care have 30 Packages to cover the Inner West Area.
