About Far West Aged Care Assessment Team (ACAT) - GWAHS
Aged Care Assessment Teams (ACAT) are teams of health professionals, such as doctors, nurses or social workers, who provide information, advice and assistance to older people who are having difficulty living at home. An ACAT can: n1. assess and approve elegibility for entry into residential aged care, in either low level care (hostel type) or high level care (nursing home type) n2. provide information about residential aged care and community care services in your local area n3. help you arrange special short-term care (respite care) so you or your carer can take a break, and n4. assess and approve eligibility for a package of community care (Community Aged Care Packages) to help you continue living at home, or refer you to other services that will help you continue living at home.nThere are ACAT delegates based in Balranald, Bourke, Brewarrina, Broken Hill, Dareton, Lightning Ridge and Walgett that cover the Far West area of NSW.
