To enable a person with a terminal illness to live in dignity, peace and comfort for the duration of his or her illness. Provides, consultative visits in office for early diagnosed Life Threatening illness clients. Nurse Visit palliative care clients in -home. Liaise with GP and Palliative Care Specialist Drs so sympton & pain management can be controlled. Palliative Care Nurse specialist to assist with pain management and Personal Carer to assist with hygiene & daily living activities. Respite In-home service. Counselling, support, Grief & Bereavement, Education & Information support, Equipment HIre service (Active Plus Hire Equipment)
