About Fremantle Community Care- (HACC)

The HACC Services provided by Fremantle Community Care is transport through Dial-a-Ride and respite care for disabled residents through Fremantle Connections and the Freo Club. A non-HACC podiatry service is also run by Fremanlte Community Care at its council offices in the Queensgate building ,Mon-Thurs and appointments need to be made between 9am-3pm.The only HACC service available is transport(dial-a-ride) .Social support for disabled adults through clubs and recreational activities. These are called Fremantle Connections and the Freo Club as part of the Outreach programme.Services available to residents in the City of Fremantle, Town of East Fremantle and Town of Mosman Park.Transport is extended to residents City of Cockburn and City of Rockingham
