Case management service for individuals with a primary physical disability - school leavers with moderate to high support needs.nTraining and support programs based on individual needs aimed at further developing the skills required for enhanced community integration and access to employment.nPrograms include: transport and access, links with education and training services, social activities and community integration, recreational activity, resources for resume development and job searching.nNot centre based, includes in-home and community support.nTransport training available.
