About Hammond Care - Extended Aged Care at Home Program (EACH)

Provides support at home as an alternative to high level residential aged care (nursing home). Support may include personal care, home help, shopping, equipment, medical and other assistance within the scope of the program. Hours of support are determined with client. If package is not suitable for client they can cross over into a residential aged care facility. Clients are assessed by ACAT. The waiting list for packages is maintained by ACAT.
