The Area HACC Manager develops and co-ordinates Home and Community Care initiatives within the Northern HACC area. Provides basic maintenance and support services to assist the frail aged, younger people with a disability and their carers to continue living within the community and to avoid premature or inappropriate referral to institutional care.
