About Home and Community Care Program - Tasmania

The Home and Community Care (HACC) Program provides grant funding to a range of government and non government organisations to deliver a comprehensive integrated and co-ordinated range of basic maintenance and support services to the target group. Organisations support people to remain as independent as possible at home and in the community thereby enhancing quality of life and/or preventing or delaying inappropriate admission to long term residential care. Eligibility is based on frailty relating to impaired functional capacity where people have difficulties with activities of daily living such as dressing, preparing meals and using public transport.
