Sub -TeamsnMilton - (02) 4455 1333nShoalhaven - (02) 4464 6951nACAT helps older people and their carers to work out what kind of care best meets their needs when they are no longer able to manage at home without assistance. nACAT provides information on suitable care options and can help arrange access or referral to appropriate residential or community care services including the Occupational Therapist assessor. nSelf referral or referral from other services. nThis assessment is required for entry into aged care facilities and access to community care packages. nThere is a Multi-Cultural Aged Care Consultant on staff. nAssessments are valid for one year across Australia, and takes up to 2 hours in the clients home.
