About Kesher Care Packages

The Kesher program provides Frail older people (and younger people with disabilities) with a case manager to help them access services and to co-ordinate their total care with CACP, EACH, EACH Dementia and Linkages packages. These packages enable recipients to maintain their independence and remain in their homes as well as participate in a variety of community activities. A case manager visits applicants to conduct assessments and reviews; and plan and coordinate services (eg, domestic assistance, personal care, respite care, meals, physiotherapy, nursing care etc) for the person in a flexible and person-focused way with particular focus on the integration of community and health services. Clients must be Jewish and live in Glen Eira, Port Phillip, Bayside or Stonnington.
