About Lady Davidson Hospital Inpatient Aged Care Assessment Team (ACAT)

The Inpatient Aged Care Assessment Team consists of Medical practitioners, social workers, and other health professionals. The team assesses the individual care requirements of each person. Following the assessments, the team in consultation with the patient and family will make recommendations that may include referral to local community services, Community Aged Care Packages, coordination of alternate accommodation such as hostel type or nursing home placements. The service is for people who are current patients of Lady Davidson Hospital.
