About Lemongrove Community Health Centre (Complex Aged & Chronic Care)

Provides a range of health services aimed at maintaining clients in their own homes and aged day centre activities. Services available include: Home Nursing, Occupational Therapy, Physiotherapy, Speech Pathology, Counselling, Bereavement Support, Palliative Care & Aged Care Assessment team. Community Nurse referrals for home visits on weekends, to commence over a weekend, or those required over a public holiday period need to be received by 12 midday on Fridays or 12 midday on the last working day prior to a public holiday. All new referrals must be made through Central Intake on 1800 013101.
