About Lower North Shore - Aged Care Assessment Team (ACAT)

Aged Care Assessment Teams consist of doctors, social workers, and other health professionals. They complete assessments of the care needs of older people, which are valid for one year across Australia. Following the assessments, the teams may refer clients to one of the following options: local community services; Community Aged Care Packages; nursing homes or hostel type care. Assessments for emergencies, or for younger people suffering from age related illnesses, are responded to individually.
