This service provides support coordination and direct services through the following programs: 126 CAPS Packages, 15 EACH Packages, 50 COPS Packages. Case Management under the Home and Community Care Program. 12 DEMENTIA RESPITE SERVICE (DRS) places for carers of people with dementia who have challenging behaviours. 5 CONNECT places, short term Case Management intervention service offering support planning and coordination to younger people with a disability, particularly people with acquired brain injury. CoMPacks, short term support for people leaving hospital - includes direct in-home service, support coordination and referral.
