The program provides the opportunity and resources for older people who are discharged from hospital to regain confidence in daily living skills. The aim is to promote and achieve maximum independence. The program provides care and rehabilitation in a 15 bed unit and in your own home.nA care plan will be developed in consultation with you and your family to establish your goals. You will have access to a case manager, specialised nursing, physiotherapist, occupational therapist, speech therapist, dietician, geriatrician and social worker.
