State-funded community-managed psychiatric psychosocial rehabilitation and support service; focus on supporting people with mental health issues who are homeless or at risk of homelessness to live independently in the community. Consumers and staff work together to design the best plan for the consumer's recovery. All referrals need to come from the consumer?s mental health key worker
