About Ozcare Outreach and Support Program

Gold CoastnOzcare Outreach and Support ProgramnProgram provides services to eligible people (a person with a disability, including mental health, intellectual, physical, drug & alcohol associated issues and the frail aged) who are homeless, at risk of becoming homeless and people who reside in supported accommodation facilities, boarding houses, caravan parks, hostels, motels, Department of Housing (where tenancy is at risk). The aim of the program is to assist clients to access mainstream health services by providing care coordination and support services. We aim to support clients with service provision to assist them to maintain their accommodation or find alternate accommodation should they be inappropriately placed.
