About Prince of Wales Hospital - Dept of Geriatric Medicine

The Dept aims to care for the elderly in the hospital and for out patients on a referral basis. The department provides domiciliary home visits for patients on a referral basis. The Dept works with the Aged Care Assessment Team and is currently undertaking studies in the nature of memory disorders. Post acute care services are available for patients, that assists in planning services for discharged patients who are re-entering the community. The Dept also includes a team of community health professionals who are responsible for providing support services for patients who are living in the community.
