About Rockdale Community Mental Health Centre - Case Management Team

Case Management Teams work at the Community Mental Health Centres to offer ongoing treatment and support. This is mainly to people with long-term mental health disorders such as schizophrenia and mood disorders. The services offered by these teams include: Bilingual and bi-cultural services including counselling in Greek, Arabic, Cantonese and Macedonian (Interpreter services are available for other languages}. Case management. Counselling. Education about mental health and illnesses. Home visits. Rehabilitation. Supervision with medication. Support services for the consumer, their family and/or their carers. The intake for this service is through St.George Hospital Central Intake and services may be provided at the hospital or other local community health centres.
