Services include -n- District Nursing Service (HACC) - provides nursing care in peoples homes, schools and workplaces 24hrs/7 days a week. General services include nursing care for the frail aged, the housebound, those with disabilities and the highly dependent; post-acute nursing care; at-home care for the terminally ill; complex technical care; assessment and case management; physiotherapy support; medication management; ethnic liaison services; and the Homeless Persons' Program. n- District Nursing Service Specialist Program (HACC) - Specialist programs include palliative care, breast cancer management and support, diabetes management, continence management, specialised aged care, wound management, HIV/AIDS treatment, home-based chemotherapy, intravenous antibiotic therapy, hospital in the home, lymphoedema management, cystic fibrosis treatment, stomal therapy, linkages community aged care program and case management, haemo-oncology and support for the homeless.
