An extended care case management service for people who are aged, frail or have disabilities and have high support needs. nIn order that they may remain in their own housing, the service provides home help, home maintenance, advocacy, emotional support, social activities, emergency assistance, nursing services and assistance with daily living activities and meal preparation.
