Provides assessment, information, advice, referral and assistance to older people and people with disabilities who wish to remain living in the community with support or who are considering moving into a Residential Aged Care facility. Services proofed by this service:n* Aged Care Assessment Team (ACAT)n* Hydrotherapyn* Medical Clinics (Outpatient Clinics in Geriatric and Rehabilitation Medicine, Post discharge from the Dixson Unit Inpatient facility)n* Occupational Therapy (OT) Community/Outpatientn* Physiotherapy (Community and Outpatient)n* Podiatry n* Speech Pathology (Community/Outpatient)n* The Dixson Day Activity Centre operates on Wed, Thurs and Fridays between 10.30am-2.30pm and is located within the grounds of the Royal Rehabilitation Service Sydney (RRCS).n* Additional services include an Equipment Loan Pool for the short-term loan of equipment.
