The role of this service is to improve quality of life and prevent re-hospitalisation of patients with Chronic Obstructive Pulmonary Disease. Referral from all health professionals.nEducation of clients and carers regarding symptom management: risk factors, signs and symptoms.nAssists with helping clients understand their medications and to take them as prescribed.nLiaison between the GP/specialist and the client to enhance communication.nParticipation is voluntary and at no cost to the client.nRruns twice weekly supervised exercise, education and discussion regarding diet, exercise, medications and lifestyle issues.
