Community Aged Care Packages are planned and managed packages designed for each individual and are based on their particular needs.nThere are x69 CACPs (Gen , Dem , ATSI ,CALD) and x20 EACH (Gen , Dem).nThe PEP ( Personal Enablement Program)nPEP is a home based program which operates across the Perth metropolitan area and in selected country areas. It is a HACC funded. During PEP your normal home care services may continue or Silver Chain will either provide the services or work closely with your current provider.n PEP runs for up to eight weeks and is delivered by a multidisciplinary team comprising of:nRegistered nurses. nOccupational therapists. nPhysiotherapists. nTherapy assistants. nCare aides (personal care). nDomestic assistants.nThe PEP program aims to:nAssist you to achieve your highest level of independence. nReduce or eliminate your need for ongoing services or readmission to hospital. nPrevent or delay any functional decline and promote healthy ageing.
