Home Independence Program (HIP) is an innovative service that aims to improve your overall health and independence if you are over 65 years of age. Personal Enablement Program (PEP) assists you to regain and boost your well being after a period of time in hospital. PEP supports your gaining independence in daily living skillls.nPEP is a home based program and aims to reduce or eliminate your need for ongoing services or readmission to hospital ; and prevent or delay any functional decline and promote healthy ageing . Runs for up to eight weeks and is delivered by a multidisciplinary team , including :-nEducator.nRegistered nurses. nOccupational therapists. nPhysiotherapists. nTherapy assistants. nCare aides (personal care). nDomestic assistants.nSilver Chain also operates Continence and Diabetes Clinics plus community and palliative care nursing . HIP and PEP are funded by HACC
