Assessment Teams can help you obtain a range of Commonwealth funded services to help you continue living in your own home, or enter a residential care facility such as a nursing home or hostel. The assessment is in a consultation with one or more members of your Assessment Team. The aim is to help you choose the available services that most suit your identified needs. The Team will give you all the information you need about what happens at an assessment. They will explain the assessment results and what they believe is the best type of care to suit your needs.
