Commonwealth Carelink Centres are information centres for older people, people with disabilities and Service Providers who provide care and services. Centres provide free and confidential information on community aged care, disability and other support services available locally, interstate or anywhere within Australia.nnSome of the community information service include providing information of organisation which supports older people and people with a disability to remain living independently in their homes and avoid premature entry into aged care facilities. Some of these organsations are Aged Care Assessment Team (ACAT), Community Care Aged Packages (CACP), Home Care (HACC) Services, Allied Health Services, GPs, Accommodation, and other services to meet an identified need.
