About Southcare - Sutherland Aged Care Assessment Team (ACAT)
The Sutherland ACAT is comprised of Registered Nurses and Social Workers who assist frail older people to gain access to the types of services most appropriate to meet their care needs. This includes determining eligibility for some Commonwealth subsidised age care services, including residential aged care and Community Aged Care Packages (CACPs). A comprehensive assessment is undertaken and the available service options, to meet their individual needs, suggested. Referrals to a range of community and other Southcare services can then be made to meet their needs and preferences. The Nursing Home and Hostel Liaison Service (NHHILS) operates within the Sutherland ACAT. The service maintaining a central waiting list of Sutherland Shire residents assessed as needing nursing home care or CACPs. NHHILS also provides access to up date information on the nursing homes and hostels to prospective residents, carers and health professionals.
