Provides support services for frail aged and younger persons with disabilities, including persons from multicultural and multilingual backgrounds. Provision of multilingual staff. Community Transport Program. HACC Assessment required for most services. Also prov cross-cultural training to HACC Service Providers. Young Lifestyle Care facility for young persons with a disability who may be at risk of entering an Aged Care facility-Coordinator: Wendy Philips.
