About Support Coordination Program for Older Parent Carers - Tweed Community Options

The program aims to support older parent carers of an adult son or daughter with a disability, who lives at home, to maintain their caring role by providing support & respite opportunities, and to plan & prepare for a transition from parental care. The program provides case management and access to: flexible support linked to appropriate services, development of informal networks & peer support, assistance with planning for the future and opportunities for social & community participation.
