Assists people with disabilities to access local services and supports. Facilitates links between clients and services. Meets needs and wants that allow better and broader access to services and community, which in turn enable and promote independence and empowerment. Reduces the potential for breakdown of formal and informal supports for people with a disability. Short to medium term only. Funding is tied to the agreed outcomes with clear objectives, entry and exit strategies. Services are brokered as necessary.
