After hours doctor and an Aboriginal Health Worker for all age groups in Polyclinic. [The referral criteria for Rehab Unit is 16 years and over, Medically stable, Requiring the input of more than one health care professional eg: nursing and occupational therapy, Living in the Westlakes geographical area, able to benefit from a rehabilitation programme. The Rehabilitation unit provides assessment/therapeutic intervention, rehabilitation, education of clients , their carers and community groups, liaison referral to other community service providers.]
