About Transition Care Program (TCP) - ATA and City Views Transition Service

Jointly-funded initiative of the Australian and State Governments, collaboration between the aged care sector, public hospitals and the community for older people who require short-term restoration support following an acute hospital in-patient episode and who are at least at low level care as defined by ACAT. Flexible services are provided either in community or in a residential environment for up to 12 weeks, depending on need. Participating public hospitals: all in the metro area plus Angaston Hospital, Balaklava Hospital, Clare Hospital, Kapunda Hospital, Lameroo Hospital, Mannum District Hospital, Mt Barker Hospital, Mt Pleasant Hospital, Murray Bridge Soldiers' Memorial Hospital, Northern and Central Yorke Peninsula Community Health Service, Strathalbyn Hospital, Tailem Bend Hospital, Tanunda Hospital.
