About Uniting Care Ageing Sydney Region Community Services

Support to help older people who live alone or with a carer with day-to-day living. To find out exactly which support services an assessment will be conducted by the CACP Coordinator. Following the assessment, a Care Plan will be designed and agreed upon for services provided. The Care Plan is reviewed regularly to make sure that clients are getting the help needed. Services may include personal care, laundry, pastoral care, meal preparation, shopping, housework, social and recreational outings, transport and help getting to and from appointments, gardening. Priority may be given to older people with high levels of needs (for example people who suffer from dementia).
