Service provides Community Aged Care Packages (25)(CACP) and Extended Aged Care Dementia Packages (11) (EACHD) to eligible clients as an alternative to entering residential care. Packages are individual flexible and tailored packages of care provided to support Frail Aged clients whom wish to remain living indepenantly in their own home for as long as possible. Services may include Domestic Assistance, Personal Care, Transport to appointments, Meals, Shopping/banking assistance, Respite Care, and Gardening Assistance.
