Role of the service is to improve quality of life and prevent rehospitalisation of stroke patients.nHome follow up program to enhance carer quality through co-ordination of support, health care services and respite services.nThis service educates clients and carers regarding strokes, risk factors, signs and symptoms.nAssists with helping clients to understand their medications and to take them as prescribed.nLiaise with the client's GP and assisting them with client care.nLiaise between client and specialist/GP's to enhance communication.nParticipation is voluntary and at no additional cost to the client.nMust be referred by GP.nSupport is via phone.
