TARGETS ADULTS - Logan, disability, living skills program, Personal helpers and mentors service for people with mental illness, acquired brain injury support, social support, centre based respite, community connects program, advocacy for tenancy and assistance to locate accommodation, sport & recreation, case management. Through case managed individual and small group services CC offers opportunities for social and recreational involvement, opportunities to maintain and improve skills. Links to local community services and assistance to maintain independence, community intergration, social and recreational programs, case management.
