Central Coast Case Management Services provides individual case management for people in the HACC target group who are at risk of inappropriate or premature placement into residential care. Client needs may include a package of services. Central Coast Case Management Services - Gosford is the Compack provider in the Gosford LGA. ComPacks case management may include domestic assistance, personal care, shopping etc. The service is for people requiring two or more community services on discharge from hospital and require ComPacks support to facilitate discharge from hospital. Client must be assessed as having clinical needs capable of being jointly met in the community by a ComPack and a clinical team.
