Aged Care Assessment Team (ACAT) - Blacktown
Provides assessments of older people in order to identify client and carer needs, formulate a care plan and make referrals to appropriate community and/or accommodation services. Referrals to service preferred by telephone. Assessment by ACAT is mandatory for admission to High or Low Residential Aged Care Facility and for Community Aged Care Package (CACP) and Extended Aged Care at Home (EACH).
