Community Aged Care Packages are individually planned and coordinated packages of care tailored to help older people to remain living in their own homes. The types of services that may be provided as part of the package include personal care, social support, home help, transport to appointments, meal preparation and gardening. To receive a CACP, a person must firstly be assessed by an Aged Care Assessment Team as requiring low level care.

Hi my name is Trent. I have been unemployed since February 10th, I have applied for jobseeker but my claim was rejected and now is being reassessed by the complex review team.
