Bulli Hospital - Chronic and Complex Care - Heart Failure Service
Chronic and Complex Care (C and CC) services are aimed at streamlining and improving care of patients with chronic heart failure and better use of resources, reducing unplanned hospitalisations and improving quality of life. Service for people with diagnosis of heart failure, ventricular failure, cardiomyopathy. Action plan for treatment and ongoing care - prevention strategies. Program at Bulli Hospital of group exercises, discussion, education, counselling. A homebased service of education and prevention strategies - aim to improve life quality. Support for aged care facilities also provided. Patients can refer to the service by contacting the service coordinator, who will notify the patient's GP, cardiologist and/ or physician of their patient's enrolment.
