Camperdown Community Options - Compacks Project

ComPacks is a case managed packaged of care for up to 6 weeks after discharge from hospital. To be eligible for case managed services through ComPacks, people will be assessed as requiring two or more community services on discharge, and require ComPacks support to facilitate discharge. This service has been established for people who need two or more community services to ensure that people can return home safely with appropriate care in place. Compacks is only available through Royal Prince Alfred, Balmain and St Vincents Hospitals. The Package is designed to meet each person's assessed clinical and support needs. Services include Domestic assistance, personal care, meals , social support, centre based day care, transport and respite care. Referrals are made by Social workers, discharge planners, discharge liaison service, continuum of care coordinators, ASET case managers.
