CommuniCare 5000 is a program funded under the Mental Health Community Based Program - a Commonwealth initiative to support individuals and families impacted upon by Mental Illness. CommuniCare 5000 provides the following services; Case Management, Brokerage Funding, Support, Respite Care information, Refferral and Information, Community Education and Training. The program is available throughout the Riverina Murray, however priority of service will be given to carers and families who reside in communities of less than 5000 people.
