Community Aged Care Packages - (ACAT)
Community Aged Care Packages (CACPs) aims to provide a community alternative for frail older people whose dependency and complex care needs would qualify them for entry to low level residential care. The CACPs are individually tailored packages of care services to frail older people assessed as requiring a range of services in their own homes. An ACAT must have assessed all recipients as eligible. Service providers use a case management approach to develop and monitor care delivery to the client.

Also accomodates an Aged Care Assessment Team & Child & Family Health Service nurse. The Community nurses cover the old Holbrook Shire boundaries while the ACAT/Child & Family Health Service covers further afield.

Hello, I am a mum of two small children aged 1 and 3. I have been thinking of the value of community and would love to reach out to the older generation who may be missing their family or not have any. I'm not sure how to go about it. I'd love to take some time to visit or meet with a group of friendly people . I think it might bring some joy to some and my children will have the benefit of getting to know a different generation.
