Community Options Illawarra Inc (COPs)
COI is for frail elderly people, younger people with disabilities who are at risk of premature placement in residential care. Clients usually have complex care needs. COI offer a case management and brokered services. New services for people from CALD background and those with dementia to cover 4 LGA's. Additional services include the Compacks Program which aims to ensure people returning home after acute hospital care have the appropriate services in place to meet their needs for a period of 6 weeks. Referrals by Health Professionals from particular hospitals. The Transition Care Program is divided into 2 sections - Facilitated Discharge and Episodic Care. Facilitated discharge is for people requiring service following hospital rehabilitation. Referrals only through the Nurse Unit Manager of the Rehabilitation wards of Port Kembla and Coledale Hospitals. Episodic Care is for people who have changed needs requiring extra services for a short period of time. Referrals by anyone.
