Provides a package of support services to help people remain independent in own home and avoid inappropriate admission to a residential facility. Provides Case management of care provided (including brokerage for needed services). Packages include Community Aged Care Packages and Community Options Packages. Mobile Respite Service (respite for carers of persons with dementia - rural/remote focus) and ComPacks (case management and brokerage services) to assist early discharge from hospital - 6 week programme each client. Compacks is out of both Port Base and Wauchope Hospitals.

I’m currently in treatment at canarven street and I have been working for over a year now and it’s difficult to get to the chemist on time and to see anyone at canarven street because of my work hours, I have been put on daily pick ups until 30/11/2022 when I do a test. This was changed from weekly to daily because I came in and did a sample and it wasn’t registering a temperature or anything in my sample including methadone which is impossible so my worker assumed that I was trying it on somehow and put me on daily pick ups. It was definitely a faulty test and because I didn’t hang around to do another one, she thought the worst, I understand why but I’m working away soon and I will need a 3-4 day take out but can’t get hold of my worker no matter how many messages I leave she is always busy and never calls me back so what can I do??? I’m working away in Belfast this month
