Community Outreach Team (CORT)

The Outreach Team a branch of the Community Outreach and Therapy Services (COATS) is a community based Allied Health team. They assess and treat people who suffer from Parkinson's Disease, MS and some other progressive neurological conditions or Motor Neurone Disease at all stages of the condition. They provide short term therapy for specific goals to enhance independence and quality of life. They provide individual assessment and follow up, Parkinson's Disease education programs & intervention provided by physiotherapist, occupational therapist, speech therapist, social worker and dietician. They assess for falls prevention and management, provide swallowing and nutrition advice, counselling for emotional wellbeing, voice and speech therapy, prevention & management of contractures and advice regarding pressure care and referral to community services. Their Carers Group provides information, support and social contact with other carers. A PSP support group meets every 2 months.

Hello
My mum suffered a stroke in July and is currently in Woy Woy rehab centre. They’re hoping she’ll be home in 5/6 weeks. My dad is looking for someone to come in the mornings and evenings to help with mum. We are looking at between 4-6 hours per day. We’ll know more in 5/6 weeks as to the role itself but it’s standby assistance.
