Continuing and Extended Care Team (CONNECT)
The Continuing and Extended Care Team (CONNECT) provides ongoing treatment, support and rehabilitation, mainly to people with long-term mental health disorders such as schizophrenia and mood disorders. The services offered by this team include: cognitive behavioural assessment, co-ordination of care with other health professionals and agencies, education about mental health and illnesses, information about community resources, older aged mental health programs, programs for consumers with a mental health problem and a substance use disorder, relapse prevention, support for families and treatment for depression, anxiety and personality disorders.

My mum suffered a stroke in July and is currently in Woy Woy rehab centre. They’re hoping she’ll be home in 5/6 weeks. My dad is looking for someone to come in the mornings and evenings to help with mum. We are looking at between 4-6 hours per day. We’ll know more in 5/6 weeks as to the role itself but it’s standby assistance.
