This HACC service has an office in the Hospital. HACC is available for older people in the community and they can be referred to the Allied Health Services when required. The HACC services offerred are :- Assessment ; Centre-based Day Care ; Client Care coordination ; CSIA ; Domestic Assistance ; Home Maintenance ; Meals (home) ; Nursing Care(home) ; Personal Care ; Respite Care(in home) ;Social Support and Transport. Clients can self refer. Covers Denmark (Town & Shire), Nornalup and Youngs Siding.
